Mission
Our mission is to enhance the human experience within support systems through education, personalized coaching, and expert consulting. We aim to Refresh, ReFocus, and ReEngage the people who operate our healthcare systems to ensure future sustainability.
Western Prairie Human Services (WPHS) has secured funding through the PrimeWest Community Reinvestment Project. This funding allows us to implement tailored programs for providers in Pope, Grant, and Traverse counties.
The Western Unity Project targets healthcare staff and provider organizations across various settings, including Long Term Care facilities, Assisted Living Facilities, Home Care Agencies, Adult Foster Care Homes, In-Home Services, Community Residential Services, Clinic and Hospital Systems, Health and Human Service Providers, and Community Service Providers and their Volunteers. Additionally, we offer education and support to individuals and families living with Dementia, Disabilities, Mental Health issues, and Chronic or Terminal illnesses.

Each provider will receive specialized education tailored to meet the needs of leadership, frontline staff, and clinic systems, with the aim of informing, supporting, and educating.

Each provider will benefit from three personalized coaching visits, offering adaptive support to align with their unique needs.

For each staff member that attends 2 Virtual Sessions and 1 In-Person Session, the Prime West funding will reimburse providers or staff $150.
